New Clients: for your convenience, you may enter your information electronically or download the forms below to handwrite your answers and mail or email them to us in order for us to begin your research. Then please do the same for step 2: Work Order and step 3: Material Release below.

    New Client information

Wide: Gather all facts and details of each generation before moving back to previous generations. Deep: Prioritize the tracing of ancestry as far back as possible.

Click the "Family Trees" link above to view samples of family trees for one person or for a couple, also known as a butterfly tree.
Can you provide originals or copies of anyone's birth, baptism, marriage or death records? Do you have newspaper clippings, immigration, military records or anything else to document any of your ancestors?
If you'd like us to contact Uncle Henry (or whomever) directly to ask for information, photos, etc. please provide their names and contact information here.
Wide: Gather all facts and details of each generation before moving back to previous generations. Deep: Prioritize the tracing of ancestry as far back as possible.

If you would like us to refer to and verify any online family trees, please provide the username and password for each. We will NOT make any changes to any trees you have already created. We will simply use this information as a springboard in our research and if able to verify the validity of it, include it in the tree we build for you. This helps us to maximize the hours you have paid for if we do not have to hunt down information you already have access to.
Many times these ideas are handed down through the generations, but no one knows how you may be related to this person. Is there such a legend in your family and do you want us to try to verify it?
Can you provide this or do you know by whom and when it was written?
There are times when we need to order a record from a courthouse or other repository for which there may be a lookup fee. For instance, purchasing a death certificate might yield valuable information about that subject’s parents, location and year of birth, etc. This is usually under $10. Do you authorize these expenses for our reimbursement when the information we seek does not appear to be otherwise available?
It happens in every family. Not that we will rewrite history, but for whatever reason, please let us know if there are ancestors you don't care to research.
Identify any ancestors who were particular favorites or who for some other reason should receive extra attention.
Personal remembrances, copies of old letters from loved ones, portraits Great Grandma may have that you do not all add to your personal family history. We can do the interviews for you or provide you with a form to jog the memory of anyone who might provide clarity to your story. Just enter their names and contact information.
Even if it will take you time to locate these images, please let us know which ancestors might have portraits somewhere among your family.

For each of the fields below, enter as much as you know FOR SURE about the Birth, Marriage and Death dates and locations, residences, schools, jobs, siblings, etc. for each person. If you have a GUESS about one of these facts, put it in parenthesis with a ? Even if you don't ultimately include these siblings in your final product, it helps to ensure that we have located the right family in records when we know as much as possible about them.
